We have created this short video clip (8 mins) to act as a refresher if you completed the MA Form training but need to recap on good practice when submitting a form. (It is not designed to replace the full training.)
NOTE: This is a multi-faceted form for submission of a range of information. It is NOT a substitute for a safeguarding referral(s) and should not be utilised for referral purposes. Any safeguarding referrals should continue to be submitted to the relevant safeguarding hub. However, consideration should be given to an MA form also being submitted to the Police, detailing any perceived useful and valuable information contained within the referral(s). Similarly, criminal activity should be reported via 101, or 999, as apprioriate to the sitation.
